I just did by following the instruction on Ubuntu website but then the system refused to boot.
Later I found that it's a problem on grub, and by following the instructions here:
http://ubuntuforums.org/showthread.php?t=1581099
The problem was solved finally.
By sticking to LTS should be better but then upgrading between LTS should be more carefully since you're jumping between some major releases.
